LTC ACO is a Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO) for Medicare Beneficiaries who reside in long-term care (LTC) facilities. The ACO’s participating physicians and clinicians work hard to provide better, more coordinated care to the Medicare Beneficiaries they treat in the LTC facilities. Medicare Beneficiaries continue to receive all benefits covered by Original Medicare, from any Medicare provider of their choice, and their benefits are not limited because the physician is a part of the ACO.
Participation in LTC ACO presents a new opportunity to participate in a model that works towards achieving better healthcare and quality outcomes for long-term care residents and incremental revenue opportunities for providers. In the event quality and participation outcomes are met, participating physicians will share in the Medicare program savings attributed by CMS to the ACO.
Participation in LTC ACO creates a new and effective opportunity for long-term care facilities to be financially rewarded for improving the quality and cost of care delivered to their Medicare fee-for-service, long-term care residents. LTC ACO participating long-term care facilities have access to comprehensive Medicare claims data for ACO-assigned beneficiaries and are therefore able to understand and address the Medicare needs for their long-stay residents in a way they weren’t able to do so previously.
