Committed to Advancing the State of Healthcare for Residents in LTC Facilities

Get to Know Us

LTC ACO is the first Accountable Care Organization to serve Medicare beneficiaries who live in long-stay facilities. Since 2016, we have focused on this high needs population which includes the frail elderly, individuals living with Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure and those with multiple chronic diseases and complications.

LTC ACO is a pioneer in leveraging the benefits of an accountable care organization in the long-term care industry and is deeply committed to the success of its participating partner practices. By participating in LTC ACO, primary care clinicians and long-stay facilities are empowered by an expert team committed to helping them advance the state of healthcare for residents in their care.

We empower long-term care clinicians and facilities to deliver value-based care by analyzing and providing actionable data and insights that drive improved quality and cost performance. Additionally, unlike other managed Medicare programs, such as Medicare Advantage Institutional Special Needs Plans (I-SNPs), LTC ACO partner practices have NO downside risk and NO capital contribution is needed to participate.

Our mission is to enable the success of our partners in value-based care, by continuously delivering the most innovative and effective value-based care insights and interventions to our partner practices. Using this approach, it is our vision to transform the quality of healthcare for people residing in long-stay facilities with preferred, industry-leading value-based care solutions.

Today, LTC ACO has a geographic reach that includes...
