Committed to Advancing the State of Healthcare for Residents in LTC Facilities

Get to Know Us

LTC ACO is the first Accountable Care Organization to serve Medicare beneficiaries who live in long-term care facilities. Since 2016 LTC ACO has focused specifically on the special needs of the frail elderly, individuals living with Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure and those with multiple chronic diseases and complications.

LTC ACO is a pioneer in leveraging the benefits of an accountable care organization in the long-term care industry and is deeply committed to the success of its participating providers. By participating in LTC ACO, physicians and long-term care facilities are empowered by an expert team committed to helping them advance the state of healthcare for residents in their care.

We empower long-term care physicians and facilities to deliver value-based care by analyzing and providing actionable data and insights that drive improved quality and cost performance. Additionally, unlike other managed Medicare programs, such as Medicare Advantage Institutional Special Needs Plans (I-SNPs), LTC ACO providers have NO downside risk and NO capital contribution is needed to participate.

Our mission is to dramatically improve the quality and cost of healthcare delivered to these Medicare Beneficiaries, rewarding participating providers for achieving these outcomes. Using this approach, it is our vision to revolutionize the way healthcare is provided to Medicare Beneficiaries residing in long-term care facilities.

Today, LTC ACO has a geographic reach that includes...
