Transforming the quality of healthcare for people residing in long-stay facilities with preferred, industry-leading value-based care solutions.

LTC ACO is an Enhanced Track Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO) for Medicare Beneficiaries who reside in long-stay facilities.  The ACO’s participating clinicians work hard to provide better, more coordinated care to the Medicare Beneficiaries they treat in the LTC facilities.  Medicare Beneficiaries continue to receive all benefits covered by Original Medicare, from any Medicare provider of their choice, and their benefits are not limited because the physician is a part of the ACO.

Primary Care Practices

Gains for your patients means gains for you.

LTC ACO works to dramatically improve the quality and cost of healthcare delivered to residents of long-stay facilities through engagement with practitioners. Joining LTC ACO presents an opportunity to participate in a model that works towards achieving better healthcare and quality outcomes for long-term care residents and incremental revenue opportunities for clinicians.

Get rewarded for improving Medicare-covered services provided to your LTC residents.

As the first ACO focused solely on residents of long-term care facilities, LTC ACO and its participating practices can engage in the savings that accrue to the Medicare program earned by reducing unnecessary and preventable hospital and emergency room admissions. Studies show that hospital expenses account for nearly 40% of Medicare spending on patients who live in long-term care facilities¹ and that a significant portion (30-67%)² of those hospital admissions are avoidable.

Improving the coordination of your Medicare benefits and primary care practitioner to deliver better outcomes for you.

If you or your loved one has Original Medicare and your primary care clinician decides to coordinate with other healthcare providers through LTC ACO, you’ll benefit because they’ll work together to get you the right care, at the right time, in the right setting, whether that is inside or outside of the long-stay facility where you reside.
